Bariatric Surgery: A Medical Solution That Goes Beyond Weight Loss

Bariatric and metabolic surgery are often misunderstood as cosmetic shortcuts, but as experts at Damansara Specialist Hospital explain, these procedures can reverse serious illnesses and offer patients a renewed, longer-lasting quality of life.

When we think about weight loss, most of us imagine diets, exercise, or cosmetic procedures. But for many Malaysians struggling with obesity, the problem runs deeper, affecting not just appearance but overall health.

That is where bariatric and metabolic surgery step in, offering not just a slimmer figure but a second chance at life, free from diseases such as diabetes, hypertension, and fatty liver.

Prof. Dato’ Dr. Haron Ahmad

We spoke to Prof. Dato’ Dr. Haron Ahmad, Consultant Hepatobiliary and General Surgeon at Damansara Specialist Hospital’s Weight Management Centre, on why this procedure is far more than just weight loss.

“When we reduce weight through surgery, we are not just changing body shape. We are treating the whole metabolic system,” he said. “Obesity affects hormones that control blood sugar, cholesterol, and blood pressure. When the weight goes down, these metabolic diseases also improve.”

Bariatric and metabolic surgery involve procedures that help patients lose weight by modifying the stomach or digestive tract, such as gastric sleeve or gastric bypass. But it is not simply about eating less. It is about helping the body work better.

“By improving insulin sensitivity and restoring hormonal balance, these surgeries treat the root cause of many chronic illnesses, not just their symptoms,” Dr. Haron explained.

He also addressed a common misconception that bariatric surgery is like cosmetic procedures like liposuction or tummy tucks.

“Liposuction removes fat under the skin, and that is cosmetic. Bariatric surgery targets visceral fat, the fat surrounding your internal organs,” he said. “That is the dangerous fat linked to heart disease, diabetes, and other serious health problems. This is not about vanity, it is about survival.”

Not everyone is eligible for the procedure. Qualification depends on Body Mass Index (BMI) and whether the patient has related medical conditions.

“Generally, patients with a BMI above 30 who also have diabetes or high blood pressure may qualify,” said Dr. Haron. “Even those who are not extremely obese but have metabolic diseases could still benefit.”

Before surgery, patients undergo detailed medical, nutritional, and psychological assessments to ensure they are ready both physically and mentally.

He also highlighted warning signs that should not be ignored. “If you are gaining weight easily, feeling tired, having trouble sleeping, or noticing rising blood sugar or cholesterol, those are red flags. It means your metabolism is not working properly,” he said.

While the procedures are generally safe, Dr. Haron reminded that there are still small risks. “The complication rate is about one percent, but what is more important is what comes after, such as following dietary advice, exercising, and committing to long-term lifestyle changes,” he shared.

Some may think surgery is the easy way out, but Dr. Haron disagreed. “It is not easy; it is a commitment. The surgery helps patients lose weight but maintaining it takes effort. Many tell me they wish they had done it earlier.”

Mental readiness, he added, plays a key role. “You have to want to be healthy. This is not about fitting into smaller clothes. It is about living longer and feeling better.”

In Malaysia, awareness about bariatric and metabolic surgery is steadily increasing. “People are starting to understand that obesity is not just about looks. It is a medical condition that can be treated,” said Dr. Haron.

This is not just a surgery. It is the start of a healthier, more confident life.
